Health Waiver

Health Waiver for DUAL Fun Run By ticking the box below I hereby acknowledge and agree to the following terms and conditions in consideration for being permitted to participate in the DUAL Fun Run: - Assumption of Risk: I understand and acknowledge that participating in the DUAL Fun Run involves inherent risks, including but not limited to, risks of injury, illness, or even death, which may result from my participation in the event. I voluntarily assume all such risks. - Release of Liability: I hereby release, waive, discharge, and covenant not to sue DUAL Strength and Fitness, its owners, employees, agents, representatives, successors, and assigns (collectively referred to as "DUAL"), from any and all liabilities, claims, demands, actions, or damages of any kind arising out of or related to my participation in the DUAL Fun Run, including but not limited to, personal injury, illness, or property damage, whether caused by the negligence of DUAL or otherwise. - Acknowledgement of Responsibility: I acknowledge that I have read this waiver and fully understand its terms. I understand that by participating in the DUAL Fun Run, I am assuming all risks associated with such participation, including risks that may arise due to the negligence of DUAL or others. I understand that I have the opportunity to consult with a physician or other healthcare provider regarding my participation in this event, and I have either done so or knowingly and voluntarily chosen not to do so. - Fitness to Participate: I affirm that I am in good physical condition and have no medical conditions that would prevent me from safely participating in the DUAL Fun Run. I certify that I am sufficiently fit and healthy to participate in this event, and I have not been advised otherwise by a qualified medical professional. - Indemnification: I agree to indemnify, defend, and hold harmless DUAL from and against any and all liabilities, claims, demands, actions, or damages of any kind, including attorney's fees and costs, arising out of or related to my participation in the DUAL Fun Run.
